  1. The following nominee, either an individual or organization, has demonstrated an outstanding or distinguished accomplishment which supports the District's mission of providing high quality leisure facilities and/or programs to its residents on a voluntary basis:
  2. Please submit Nomination Form and Letters of Support to by Wednesday, December 16, 2015.

  4. List activities and significant achievements warranting nomination for outstanding contribution to the District and the community. Please provide adequate information. It is recommended that a minimum of 300 words be submitted. A letter may also be submitted or substituted in place of filling out "Nomination Accomplishments". If submitting a letter, please state "Letter Attached".
  5. Upload of letter optional which replaces filling out the Nomination Accomplishments field above.
  6. Please upload photos to support the individual or organization's volunteer efforts.
